In Formation Design has undertaken several projects based on reporting and summarizing the results of the Ontario Heart Health Program, a program sponsored by the Ontario provincial government. Deliverables included a poster displayed at a conference in Milan, Italy, an 8-page summary report (sample below) distributed to policy makers and programmers responsible for disease prevention in Ontario, and a PowerPoint presentation (sample above) delivered at a conference in Lake Tahoe.
